Special Olympics Registration Directions

WW-P Regional School District uses online registration for Special Olympics using a secure site called Family ID. Clink on the link on the left hand side of the page to review registration dates for our 2016-2017 sports.

Family ID registration: To set up or sign into your secure Family ID account, click here to go to the Family ID web site.

If you need to call Family ID: 1-888-800-5583, extension 1; support is available seven days a week.

Registration Directions: Once on the Family ID web site, please follow these instructions to register for Fall, Winter, and Spring sports:

  • You will receive an e-mail from Family ID with a link to confirm your new account.

  • Click on the link in your e-mail confirmation.

  • Fill out the information requested on the form and submit.

  • After your sign-up is complete, you will receive an e-mail from Family ID confirming that your sign-up was successful.

  • Log into Family ID at any time to update your information and check your registration.

  • Once you are logged in to Family ID, scroll to the bottom of the WW-P Family ID page and click on the link for Special Olympics.

  • Once in the description page for Special Olympics, click Register.
  • Once you have filled out all of the required information on to register for Special Olympics, click “Save an Continue”.  PLEASE NOTE: In order to finish the registration process you must submit a Pre-Participation Evaluation (physical form) and a Special Olympics New Jersey Medical Form to Jennifer Davis two weeks prior to the start of the season.

  • Once your athlete has been approved you will receive an email with an APPROVED status.

  • Please remember the deadlines for each sports season. Failure to do so will affect your child’s ability to participate. The registration is not complete until the necessary medical forms, inclusive of the WW-P Athletic Physical and Special Olympics Medical Form, are filled out, signed and submitted to the nurse. For our Out–of- District athletes, the physicals go to the school that would be their home school.

The school physician will review and sign off on all physicals. Due to the added time required, it is imperative that the physical exam form be submitted by the posted deadline. Physicals handed in late will result in delayed participation. The form can be downloaded and printed from this web page by clicking on ‘Forms’ on the left hand side. Please note that physicals may not be faxed or emailed.

Additional Required Forms: In addition to completing your registration on Family ID, students must complete the Special Olympics Medical form to participate in sports. The form can be downloaded and printed from this web page by clicking on ‘Forms’ on the left hand side.

Important Information about Required Forms: Please note that the online registration process does not change the requirement to submit health and medical information directly to the Nurse’s Office of your child’s home school. The WW-P Athletic Physical Exam Form must be filled out, signed, and submitted in paper copy to the nurse by the deadline of each sports season. The Special Olympics Medical Form must be submitted to a Special Olympics coach -- Jennifer Davis, Amanda King, or Alex Campbell -- in accordance with its bi-annual deadline of each sports season.
